MEDICAL SOPs
Standard operating procedures and reference material for medical and emergency services.
1. Mission & Scope
Zentra Medical Services provides emergency care, field stabilization, transport, hospital treatment, and medical roleplay for all persons without favoritism. Medical personnel are not law enforcement. Their authority is medical: patient care, treatment decisions, hospital operations, and scene medical safety once law enforcement has secured an active threat.
- Treat patients based on medical need, not criminal status, faction, friendship, or department.
- Preserve life and create meaningful medical RP rather than rushing every patient through identical treatment.
- Maintain patient confidentiality except where disclosure is required by Zentra law, immediate safety, or approved department procedure.
- Do not interfere with active police tactics. Police secure dangerous scenes; EMS controls medical treatment once safe access is provided.
2. Ranks & Responsibilities
3. Loadout & Vehicles
3.1 Standard Medical Loadout
- Flashlight
- Medical bag
- Bandages and basic treatment supplies
- Medkits / advanced treatment supplies appropriate to rank
- Radio/communication equipment required by the server
- Other medical tools assigned through certifications or hospital systems
3.2 Vehicles
- Use only vehicles authorized for your rank/certification.
- Do not loan rank-locked department vehicles for convenience.
- Air-Med and specialized vehicles require the relevant certification.
- Personal EMS vehicles, if permitted, must still follow department appearance, duty, and emergency-driving rules.
4. Chain of Command & Discipline
Routine issues should be handled by the nearest appropriate supervisor. Members may bypass the chain for harassment, sensitive misconduct, conflicts involving the supervisor, or urgent safety concerns.
4.1 Progressive Discipline
Zentra Medical uses a true three-strike framework: a third active strike triggers termination review and may result in termination. A fourth strike is not required. Eligible strikes may be reviewed after 30 days; removal is discretionary and should be documented.
5. Scene Operations
5.1 Patient Assessment
- Identify yourself and confirm scene safety.
- Assess responsiveness, airway/breathing/circulation, major bleeding, mechanism of injury, and visible injuries using the server's medical mechanics and RP.
- Minor injuries may be treated on scene when appropriate.
- Major injuries — including significant head trauma, fractures, severe vehicle collisions, stab/gunshot wounds, or unconsciousness — normally require hospital transport after stabilization.
- Explain treatment to conscious patients and create reasonable roleplay around procedures.
5.2 Value of Life
EMS must not enter an active violent scene simply because someone is down. Stage at a safe location until law enforcement or the controlling scene authority confirms access. If threatened at gunpoint, prioritize survival and comply with reasonable commands while using panic/dispatch tools only when realistically safe.
6. Police Scenes & Patients in Custody
6.1 Police Scenes
- Announce EMS arrival and stage away from an active threat.
- Enter only after the scene is declared safe enough for medical access.
- Law enforcement controls perimeter/security; EMS controls medical treatment decisions.
- Officers may assist with basic care when requested by qualified EMS, but should not direct clinical treatment.
6.2 Patients in Custody
- Confirm custody status with the escorting officer.
- EMS should not remove police restraints unless medically necessary and coordinated with the officer; emergency life-saving care takes priority.
- When privacy is medically appropriate, request reasonable space while maintaining officer safety/security.
- Document pressure to skip necessary treatment and report serious interference through EMS leadership.
- Once medically cleared, custody/escort remains the responsibility of law enforcement.
7. Hospital Operations
- Nurse or higher should respond to hospital walk-ins when available.
- Major surgery should be led by a Surgeon when available; Doctors may handle minor procedures within their role.
- Use appropriate PPE and preparation for surgery/advanced procedures.
- Active violence on hospital grounds must be reported to law enforcement; EMS should not act as armed security.
- Use private treatment rooms when practical for examinations, sensitive discussions, and custody patients.
8. Ride-Alongs
Ride-alongs may be approved by Nurse rank or higher, subject to department availability and safety. They exist to introduce potential members to EMS, not as a social perk.
- Ride-along participant must provide character identification requested by EMS.
- Serious recent violent criminal history, active warrants, or a documented safety risk may disqualify a participant.
- Gang/organization membership by itself is not an automatic disqualifier; the decision should be based on actual safety, active criminal status, or conflict concerns.
- Participant may not interfere with care, access restricted supplies, or enter unsafe scenes.
- EMS may end the ride-along at any time for safety or conduct.
9. Ethics, Confidentiality & Corruption
Corrupt-character RP may occur only when allowed by current Zentra server rules and approved department/server processes. An SOP does not grant permission to break OOC rules.
- Medical information should remain confidential unless disclosure is legally required or necessary for immediate safety.
- Using medical access to steal, free prisoners, manipulate records, supply restricted items, or target patients may result in IC investigation and OOC staff review depending on the conduct.
- IC corruption allegations may be investigated through the appropriate government/DOJ/IA structure established by Zentra.
- EMS leadership may suspend, demote, or terminate members based on documented department misconduct; server-rule violations are handled by staff separately.
10. Certifications
11. Meetings, Training & Promotions
- Department meetings should be announced with reasonable notice when possible.
- Training is required for new members and for responsibilities added through promotion/certification.
- Promotions should consider training completion, activity, professionalism, clinical RP quality, documentation, and leadership — not friendship or raw hours alone.
- High Command should document significant promotions, demotions, certifications, and discipline.
- Members on approved LOA should not be penalized for normal activity requirements during the approved period.
